Summary

BACKGROUND There is an association between PROs and clinical outcomes in symptomatic patients with PAD but their role in the subset with CLTI is unknown. We evaluated the association between PROs and clinical characteristics and outcomes in patients with CLTI.

METHODS We analyzed data from the BEST-CLI (Best Endovascular versus Best Surgical Therapy in Patients with CLTI) registry. We examined baseline characteristics and PRO scores, and stratified scores into tertiles, for each of three health status instruments, the EQ-SD, VascuQOL, and the SF-12. We examined the association of PRO scores with 12-month outcomes using Cox PH models.

RESULTS There were 1009 registry patients meeting inclusion criteria, of whom 993(98%) had at least one of the three scores available. As compared with the lower half of EQ-SD scores (<0.36). patients with high scores (0.36-1.00) were older (median age 70 vs 68 years, P<0.01) and more likely to be male (71% vs 60%, P<0.001). Specific comorbid conditions associated with lower scores, including CHF, COPD, smoking, and prior coronary intervention (P<0.05). Patients with lower ABis (median 0.59 VS 0.48, P<0.001) and higher levels of exercise and ambulatory capacity all had higher scores. Compared with patients low EQ-SD and SF-12 scores, patients with higher EQ-5D or SF-12 scores had lower MALE/all-cause mor-tality, all-cause mortality, and MACE at 1-year.
